Michael Frank Forth

Michael Frank Forth, 73, Crown City, Ohio passed away Saturday, October 29, 2016 in St. Mary's Medical Center Emergency Room, Huntington, West Virginia. Born May 22, 1943 in Huntington, he was the son of the late James William and Betty Ruth (Wentz) Forth, Jr. He was a member of Crown City Community Church and a loving husband, father and brother.
Michael is survived by his wife, Beverly Timmons Forth, Crown City, Ohio and children: James (Denise) Botelho, Sacramento, California; Michael (Bobbi) Botelho, Proctorville, Ohio; Joe (Chandra) Botelho, Oregon and Stephanie (Steven) Ward, Temecula, California; nine grandchildren and five great-grandchildren; a brother, Patrick (Ginger) Forth, Columbia, South Carolina; a step-sister, Diane Whaley, Huntington, West Virginia and uncle, John (Patty) Wentz, Lincoln County, West Virginia.
In addition to his parents, he was preceded in death by his son, Larry Forth, March 2, 2006.
Graveside services will be conducted Tuesday, November 1, 2016 at 1:00 PM in the Crown City Cemetery, Crown City, Ohio. Arrangements are under the direction of the McCoy-Moore Funeral Home, Wetherholt Chapel, Gallipolis, Ohio, who is honored to serve the Forth Family.
